Vancouver Island specifically. A friend is having trouble getting a diagnosis and is just wondering if there has been a problem with it out there as this apparently is a remote possibility.

Cryptococcus has been ruled out.

Just thought I'd throw it out here.

According to the internet, the incidence is

"Blastomycosis - prevalence
Infection is endemic in the US in the southeast and central states and in the towns bordering the Great Lakes, with the highest incidence in Arkansas, Kentucky, Louisiana, Mississippi, North Carolina, Tennessee, and Wisconsin.
Other reported areas of infection include parts of Canada (Ontario, Manitoba), Africa, India, and South America."

That's all I can tell you except that the infection areas for Coccidiomycosis and Histoplamosis (other funguses infecting lungs) do not include Vancouver Island.

Have they done titers?
