Boon's latest bloodwork :-(

Hi Folks,

Boon's latest blood work shows that he is not only not getting well but actually functioning worse in several areas; platelets and creatinine levels, in particular are worrisome. This coincides with a definite plateau of improvement of his observable symptoms. He was notably worse today as compared to two days ago in terms of lethargy and lack of zest for life.

We will be monitoring bloodwork often and back at NCSU vet school for a whole new round of tests on Thursday; they have mentioned looking, again, for a microangiopathy or vector borne disease. Many fine veterinarian minds are consulting on his very puzzling case, including the lead clinicians in several areas outside of internal medicine. I'm so hoping the new round of tests will identify a specific problem that can be successfully treated.

Appreciate all kind thoughts and prayers.

Poor Boon! He is in my thoughts and prayers. Hopefully the vets will find an answer soon. Try to hang in there.

Creatinine and platelet levels are low or high? Do you know his vwd status? Do you know if he was tested for JRD?

Hey, Dobiewankanobi, Boon is a carrier. Low platelets, high creative. What is JRD? We need to ask all kinds of questions because it's still a puzzle. I'm grateful for any thoughts!

We are here at NCSU now. They are doing regular bloodwork and a slew of other tests (non invasive).

JRD is juvenile renal dysplasia. I have known of 3 Dobes that have passed from this recently. Right around the age of 2 years. There is a test for it, in Canada. Your breeder may have had it done on the sire and dam.

The low platelets combined with higher creatinine reminds me of a friend's dobe who had a similar experience a few years ago. They never did figure out what it was. Do you know if he was tube fed? If he was smaller than the littermates?

Do you have any of the other values from the bloodwork? Phosphorous, BUN, etc? Any changes to a new food? I'm sure you've thought of all this but perhaps a little outside brainstorming might provide new ideas for NCSU to follow?

Thanks so much for all the kind posts. Boon's bloodwork yesterday showed some improvement in liver enzymes, yay! Bilirubin has dropped to normal. Also the creatinine level had dropped into the normal range. Both of those are heartening. His platelets are still low but holding steady and not dropping.

Today, he was very disinterested in food once again and that is quite discouraging. He sleeps a huge amount and just has that sad look on his face all the time. They are retesting for all the vector borne diseases using both serology and PCR methods. Also doing the month-long test for Bartonella which was developed here at NCSU vet school + testing for toxoplasmosis and several other obscure infectious agents.

This is their working theory at this time: Boon had a serious infection which caused him to be quite sick. Then he was given the hard-hitting course of doxycycline at the first visit to our local emergency room. The doxycycline caused a huge die off of cells which, in turn, created the storm affecting his liver and other organs so negatively, landing him at the vet school emergency department and requiring the week of hospitalization. They believe he has an ongoing infection which has not been eradicated by the current medication protocol so they are seeking to identify that source of infection.
