The order form came with the paperwork the evaluator gave me when we passed. He gave me the forms to send in to receive the CGC certificate, patch, and I.D. tag. The certificate and the patch haven't come yet.

Try calling 800-659-0911 and see if they can send/fax you a form to fill out and send back, or maybe order by phone. The tag cost $6.00.
