Not sure if this will help. I have a diabetic doberman and they get UTI very easy. What I have to do is have a clean sample taken and cultured. This way I know what kind of antibotics to use. When the medication is done I have another clean sample culture to make sure the infection is gone.

If your vet has never culture the urine to find the right medication it will never go away. Most vet will give you a broad spectum antibotic.

Ok, so I finally had the vet apt. The vet was incredible, spent a full 2 hours just with me. And I can't get over how much cheaper vet care and meds are in the states...I paid probably less than a third of the price then if I had taken her over here again.
SO, here's where we stand. She was diagnosed with a recessed vulva, the vet said it looked irritated but not infected. This is from the urine dribbling. The vet actually saw the droplets come out, and was amazed that she was on such a low dose of proin (40mg) and wondered why the vet hadnt' upped it. So she's going to be on 75 mg now, then if that doesn't work then we will add some estrogen pills to the mix. Keeping fingers crossed! We did a urinalysis, it was squeaky clean other than the ph was still at 8. There was not protein or anything, kidneys not an issue at this point. We did a follow up xray to rule it out, no stones at all (yes!). SO it's a great time to start her on the increased dose and keep that sphincter from getting bacteria in now that her urine is clean.
For her inflamed/scabby ear tips, she did a skin scraping and is going to send it for a fungal culture, she' didn't think that blood allergy testing was necessary at this time (she's not a frantic scratcher, paw licker, etc) so she made some dietarty suggestions. She did take a vial of blood she will freeze in case i change my mind. She also did a mite and ringworm test (neg) I'm goin to try her on the wellness core (chicken) that has few ingredients and see how that goes.
I was overwhelmed by the amount of time an effort she put in with me...what a change!! She had her books out, gave me a bunch of printouts and I almost passed out with the small bill when i left. So my girl is gonna do better, I'm taking her off the yogurt (her ears are worse since she started) and give her prebiotic capsules.
So I'm excited to see how the upped proin does, and this new food!

You have some clear idea of the problem, and a clear path forward for treatment. I think that's what we all want when we have a pet under the weather.
