Josie update

Yesterday I was concerned that Josie's abdomen looked distended, so I called to see if they could do her echo a few days early and check her out. They couldn't get me in so I went to my regular vet. He did an ecg and saw a few PVCs but otherwise she looked great. She went home, played in the yard and ate her dinner. I was sitting in bed reading and she was up against me and I had my hand on her side. I felt an irregular heartbeat so I grabbed my stethoscope and her heart rate was so fast I couldn't count it and it sounded like a pounding hammer. I rushed her to the specialist, which has emergency hours. She was alternating between a-fib and v tach, but still normal looking from the outside. They gave her a lidocaine bolus and started her on a lidocaine drip. She did go into vtach once more overnight when she got excited for her food, and received another bolus. They noted some more arrhythmias overnight as well. Her echo this morning looked about the same-poor heart function but no evidence of failure. The vet said she was very bright, social, and well-appearing. The goal today is to start her on oral digoxin and mexiletine and wean her from the lidocaine. If she does okay, she may get to come home tonight. It has been a horrid past 12 hours and I was so worried I would lose her. I think I just got lucky by feeling the rhythm since she was acting completely normal. I would appreciate any good vibes/thoughts/prayers that you have.

Saw the IG post and my heart sank at the update this morning, glad to hear you caught it in time and were able to get her into the vet's office so quickly! All fingers and dober extremities crossed for Josie's continued health here.

Wow... You and Josie sure dodged a bullet. I hope your experience becomes similar to ours.

After a very serious ventricular tachycardia episode, are oldest boy (at the time) was given an extremely poor prognosis. However, after being put on a medical regime, he lived a happy normal life for several years with no visible signs of DCM. Wonderful years...

Ultimately, he did succumb to Sudden Death, but it was mercifully quick and uneventful.

I wish you the all the best in Josie's recovery.

Thank you for your support and well wishes. Since coming off the continuous lidocaine infusion she has had two more runs of v-tach which quickly responded to a lidocaine bolus. They are keeping her another night to see how she does, and hopefully try to get her oral medication where it needs to be. Her vet sounded hopeful that we can manage this with oral meds. I'm trying not to get too discouraged at this point, but I'm kind of freaking out.
Meadowcat, she has not been holtered before. I'm kicking myself for not doing routine echoes and holtering the last couple of years because I'm wondering if I could've caught this earlier. She has just seemed so healthy.

Josie had a great night last night! She does throw some VPCs when she gets excited, but they're comfortable sending her home this afternoon as long as she doesn't have anymore a-fib or V-tach. I'm a little concerned about how excitable she is-she goes bananas when she sees a squirrel, is constantly stalking and pouncing on my foster and chasing him as fast as she can run around the yard, and in general just jumping, leaping, spinning a lot. Her vets feel like if I keep her calm that would be comprising her quality of life, so to assess her when she becomes very excited, and maybe add a beta blocker if she needs it.

I personally feel like if I were in the situation of having a dog with DCM, I wouldn't change what we do - I'd rather let my dog enjoy their activities. I know some people do modify, but I don't think it ultimately changes the outcome, and my dogs would be so much less happy if we didn't continue the things they love. So I agree with your vets.

I can't remember - Is Josie seeing a cardiologist? With a 24 hour holter to give you a clearer picture, they can really customize her medication regimen. I know quite a few people dealing with this (unfortunately) and depending on what is going on it can really change how they medicate. BUT, there are a LOT of options that can really help.

I'm sending lots of prayers your way, and am so glad she's feeling better right now.

Edited to add - hopefully you have some good breeder support right now, too. Many breeders own a holter monitor - maybe yours does and could loan it to you? Both of mine own one and will ship to me at no charge. And many of them are REALLY well educated on the latest developments and information.

My baby is home! Thank you everyone for all the thoughts and well wishes. I will work on getting some pictures of her up. She did have a few pairs of VPCs again today, but was without symptoms. She is so happy to be home.
Meadowcat, she isn't seeing solely a cardiologist but an internal medicine specialist that is very experienced in dealing with DCM. My breeder has been amazing through all of this; it's so great to have her support. She doesn't have a holter, but I've been looking at where to get them.
I'm meeting with a pet sitter on Monday that can help give her medication when I'm at work. She requires mexilitine every 8 hours so would need a mid day dose. I was told to try and keep her calm for the next two weeks until her follow-up...that's not going well, lol. She has been a wild girl. Again, thank you everyone for everything!

Here are some pictures. I've never uploaded any in the app so I hope they work. The first is the day she was hospitalized; she was at the hospital visiting patients. The other was her in her cone when I got to visit her yesterday, and the rest are this afternoon after coming home.

Well, she's back in the hospital. I noted another arrhythmia last night and took her to the ER; V-tach again. They were about to to give her a lidocaine bolus but she flipped over to a more normal rhythm. She had some abnormal beats over night but no v-tach. She was started on Sotalol, and can hopefully come home tonight.
